I. FIRST –TIME APPLICATION (CHILD BORN IN THE U.A.E.)

Please note: Report of Birth must also be accomplished at the Consulate General by the applicant's parent(s) at the same time.
  1. ePassport Application Form, accomplished by either parent (if illegitimate, by mother);
  2. Birth Certificate issued by the UAE Ministry of Health, duly stamped by the UAE Ministry of Foreign Affairs (translated into English) and 6 photocopies;
  3. Report of Birth issued by the Philippine Consulate General, and 6 photocopies;
  4. 6 Photocopies of the  Passport of each parent;
  5. Authenticated Marriage contract of Parents and 6 photocopies;
  6. Personal appearance of Filipino parent(s) and infant/child;
  7. ePassport Fee: AED 240;
  8. No need to bring passport photo as this will be taken during the Enrolment step;
  9. After processing the documents at the Consular Services Area, submit 5 copies of the documents to the Releasing section and 1 copy to the Passport Enrolment Section;
  10. New passport can be claimed 4-6 weeks from date of application;  Present receipt


II. RENEWAL OF PASSPORT
  1. Photocopies of most recent passport
    1. Inside front cover
    2. Inside back cover
    3. Page containing latest Philippine immigration departure stamp
    4. Page containing latest UAE immigration arrival stamp
    5. Page containing UAE Visa
  2. Duly  accomplished ePassport application form;
  3. No need to bring passport photo as this will be taken during the Enrolment step;
  4. Personal appearance of all applicants is required;
  5. ePassport Fee: AED 240
  6. New passport can be claimed 4-6 weeks from date of application: Present old passport and receipt

III.  AMENDMENT OF ENTRIES

  1. Passport and Photocopy of Front Inside Page
  2. Application for Amendment (form available at the Philippine Consulate General)
  3. Fee: AED 80

IV. REQUIREMENTS FOR REPLACEMENT OF LOST PASSPORT
    1. For the ePassport, all applicants are required to appear personally at the Consulate General to renew their passports;
    2. Duly accomplished ePassport application Form;
    3. Police Report on Lost Passport (with English Translation);
    4. Duly accomplished Affidavit of Loss (form available at the Philippine Consulate General);
    5. Photocopy of lost passport, if available;
    6. If photocopy of lost passport is not available, present Birth Certificate
    7. ;Fee: AED 460
    8. New passport can be claimed 4-6 weeks from date of application: Present affidavit of loss and receipt;

        Regular Processing:
    • 44 pages ePassport: DHS 240 (two hundred forty dirhams)
    1. For amendment of Passport
      • DHS 80 (eighty dirhams)
    2. Replacement of Lost Passport
      • 44 pages DHS 460 (four hundred sixty dirhams)
    VI. LIST OF SUPPORTING DOCUMENTS

    Valid and Proper Identification are the following:
    1. UAE Labor Card
    2. RP and/or UAE Driver’s license
    3. UAE Health Card
    4. OFW E-Card
    5. Company ID Card
